Comprehensive
Eye Care Our services include complete eye examinations that fully
assess the health of your eyes. Testing will be done to detect glaucoma,

your individual eye conditions. Eye Health provides a full range of treatment,

We know that
a complete discussion of your examination results and suggested treatment is
important to you. You can expect us to thoroughly inform you about the health

Fees
and
keeping them low! Eye Health participates with Medicare, Blue Cross
& Blue Shield and most major insurance companies. We gladly accept
assignment. For your convenience, our office will do all Medicare and
supplemental insurance billing. In other words we fill out the forms, not
you.
Eye Health is a member of many health maintenance organization
(HMO's). Please remember, if you are insured through an HMO or HMO Medicare
replacement plan, you must have authorization from your primary care physician
before visiting our office. Eye health will send an itemized statement once we
have heard from your insurance. You are responsible for your insurance
deductible and annual co-payments.
We realize that unexpected medical
expenses can sometimes be difficult. Please be assured that you can discuss
financial arrangements at Eye Health with complete
confidentiality.
Financial Aid - Eye health works closely with
local social agencies in providing needed eye care. We participate annually
with the American Academy of Ophthalmology's National Eye Care Project. We
continue those efforts all year long providing free cataract surgery for those
in need. |
